Why send forms before the appointment

Instead of filling paperwork in the chair, your client completes everything on their phone beforehand. You arrive prepared, consultations run on time, and every consent and signature is stored as a dated record on the client — ideal for patch tests, medical history and waivers.

1. Build your form

Go to Forms and add questions — short text, long text, multiple choice, date, a consent checkbox, or a signature field where the client signs on their screen. Or add a ready-made template (patch test, medical history, facial consultation, consent & waiver, aftercare) in one click and edit to fit.

2. Scope questions to the right service

Each question can apply to all services or just one — so a patch-test question only goes out for colour treatments, and a medical history only for aesthetics. Set this with the 'Applies to' dropdown when you add the question.

3. Send automatically before each visit

In Forms, turn on 'Send forms before appointments' and choose how many hours ahead (e.g. 48). Pick email and/or SMS. We text or email your client a private link before their appointment; they tap it, fill the form on their phone, and sign — no app or login needed.

4. See who's completed

The Forms page lists recent sends with a status — awaiting reply or completed — so you can chase anyone outstanding. Answers and signatures attach to the appointment and stay on the client's record.

Good to know

• SMS delivery of form links uses your SMS balance (email is free)
• Signatures are captured on-screen with your consent statement shown above
• You can also keep questions on the booking form itself, as before
